Our office can make it convenient and easy for you and, of course, you can count on the expertise of Dr Ginzburg who is a civil surgeon.

You can also count on meticulous attention to accuracy at Midwestern Institute of Health, Ltd. Our staff has had special training to ensure accuracy in filling out the forms to make sure they adhere strictly to USCIS guidelines. When all parts of the exam are satisfactorily completed and you’re deemed eligible, your I-693 paperwork will be completed and sealed for you to submit to USCIS.

What Tests are Performed for Your Exam?

An immigration medical examination covers key areas: your medical history, a physical examination, blood test, vaccination (if required).

Medical History

We’ll discuss your health history, including any medical conditions that you may had in past.


Physical Examination

Dr. Ginzburg will conduct comprehensive medical evaluations. He will also do a blood test as required by USCIS.


Vaccination

USCIS requires that all patients provide record of immunization. If you have an official record of vaccines given in another country, bring it with you and Dr Ginzburg will review it. If not, Dr Ginzburg can administer any missing vaccines in his office.


What you need to bring with you:

We will provide a current I-693 form for you to fill out at our office. Here’s what you need to bring with you when you come for your exam:

  • A government-issued photo identification (such as a driver's license, ID card, or a passport).
  • A record of vaccinations, if you have them.
  • The examination fees which includes the physical exam, testing for syphilis, TB, gonorrhoea
  • A list of your current medications.

What vaccinations do I need?

Some vaccines are required by the Immigration and Nationality Act, whereas others are required by the Center for Disease Control and Prevention. The vaccinations include:

  • Measles/Mumps/Rubella
  • Polio
  • Diphtheria/Tetanus/Pertussis
  • Hepatitis B
  • Pneumococcal pneumonia
  • Hepatitis A
  • Varicella
  • Influenza
  • Meningococcal

Measles/Mumps/Rubella, Varicella, Diphtheria/Tetanus/Pertussis are most commonly required vaccines. This is a current list, but may be changed in the future.
